Wow, tough question. Extremely difficult thing to do, especially if you are given a team with "individual" attitudes. I can offer some suggestions, but not one thing I say can really make the difference by itself. You will need a combination of things to happen, and some luck, for it all to fall in place.
If you win and play together, it makes teaching the "team concept" so much easier. As they say, "winning cures so many ills". They are able to see the result when great teamwork is displayed. You as the coach need to constantly preach team, team, team. Becareful how you praise and who you praise. Don't ever let your team think one player is more special than the rest, even though it may not be the case 
Share the "team" stats with your team every chance you get...team fg%, team def. fg%, team steals, team rebounds, etc. Post articles in your lockerroom on great "teams" and "teamwork", not "individuals" and "individual accomplishments".
I encourage other coaches to add their 2 cents on this difficult subject. There may be some great ideas out there just waiting for you to implement with your team.
